29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the west side of the commercial building, employees who worked without fall protection were exposed to a fall hazard of approximately 10 feet while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On the west side of the construction site, the employer did not ensure that the extension ladder was extended 3 feet above the roof area exposing the employees to a potential fall of approximately 10 feet.
