29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines was less than that permitted by requirements:      a) Job site, 907 & 909 East Carson Street, Pittsburgh, PA: On or about November 3, 2015, during construction activities, the employer did not ensure that a scaffold was located at an appropriate distance from power lines. The scaffold was set up within approximately 3 feet of the scaffold, and was not de-energized, relocated  or blanketed.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   a) Job site, 907 & 909 East Carson Street, Pittsburgh, PA: On or about November 3, 2015, during construction activities, the employer did not ensure that appropriate scaffold guardrail systems were provided, in that no midrails were installed at the ends of the scaffolding, exposing employees to falls from heights of at least approximately 19 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a) Job site, 907 & 909 East Carson Street, Pittsburgh, PA: On or about November 3, 2015, during construction activities, the employer did not ensure that employees were provided scaffold safety training while working on a scaffold. Deficiencies noted during the inspection included, but were not limited to, working on a scaffold that was too close to existing power lines and not providing mid rails along the ends of the scaffold where falls of up to at least approximately 19 feet existed.
