29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   A)    Industrial Crane & Hoist Services, Inc., conducting work at 120 Filbert Street, Elyria, OH 44036 (Elyria Foundry): On or about October 26, 2015, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, conducting repairs on the equipment such as, but not limited to, the Whiting 10 Ton Overhead Carne (Serial # 9776) located in South Melt Shop at Elyria Foundry. Employees were exposed to amputation hazards.
