29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them:        On or about 10/22/2015, while in the process of changing the hose of a Uni-mac (commercial grade) washing machine, located in the laundry department at the Wyant Woods Care Center in Akron, OH, an employee inadvertently contacted a live transformer and was injured. The washing machine was not de-energized prior to having any servicing/maintenance performed.
