29 CFR 1910.147(d)(5)(ii): If there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists. On or about September 30, 2015 and prior thereto, at the Rice Hull Specialty Products, Inc. facility located at or near 1304 AR-146 Spur, Stuttgart, AR 72160, the employer did not ensure all sources of accumulated energy were isolated or controlled, exposing employees to the hazard of being caught in the moving parts.
