29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Conference Room; A power strip was plugged into another power strip (relocatable power tap) which powered a projector and other audiovisual equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (b) Special Agents Room, Cubicle by window on left: A power strip was plugged into an extension cord which was plugged into another extension cord (daisy chain). The power strip powered computer equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (c) Special Agents Room, Middle cubicle on right: A power strip was plugged into another power strip which powered computer equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (d) Inspector Cubicle Adjacent to Tacoma Group; A white power strip was plugged into another white power strip which powered computer equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (e) District Command Cubicles: Three white power strips were plugged into each other which powered computer equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (f) Regional Director Work Area; An Amana microwave, Mr. Coffee coffee pot and a Kitchen Aid grinder were plugged into a power strip which was not rated for appliances. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances. This exposes employees to possible electrical fire hazards. (g) District Command Area; A Panasonic microwave, Vizio TV and Absocold refrigerator were plugged into a power strip which was not rated for appliances. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances. This exposes employees to possible electrical fire hazards.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage: a) Warehouse: There was a metal storage cabinet stored underneath a breaker panel which was labeled fitness center panel. b) Warehouse Open Bay; Chairs and boxes were stored in front of the breaker panels.
