29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:      3115 St. Andrews Circle, Grand Island, NE 68803 (Job-site):  The employer failed to ensure that a supported scaffold platform on which an employee was working had mud sills in use under all base plates.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.451(c)(2)(v):  The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:      3115 St. Andrews Circle, Grand Island, NE 68803 (Job-site):  The employer failed to ensure that employees that were working on a supported scaffold platform being used with a Genie forklift had the scaffold secured to the forklift.        Abatement certification and abatement documentation is required for this violation. The abatement certification sheet is enclosed.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     3115 St. Andrews Circle, Grand Island, NE 68803 (Job-site): The employer failed to ensure that adequate fall protection was provided for employees working on a supported scaffold that was over 10 feet above the ground which was attached to a Genie forklift.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
