29 CFR 1910.95(g)(1): The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels.     a. An employee in the production area was exposed to continuous noise at 259% of the permissible daily dose level, when measured with a 90 dB threshold, or equivalent sound level of approximately 96.9 dBA for the 460 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 20 minutes not sampled.     b. An employee in the production area was exposed to continuous noise at 366% of the permissible daily dose level, when measured with a 90 dB threshold, or equivalent sound level of approximately 98.7 dBA for the 457 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 23 minutes not sampled.      c. An employee in the production area was exposed to continuous noise at 68.6% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 87.3 dBA for the 453 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 27 minutes not sampled.     d. An employee in the production area was exposed to continuous noise at 73.4% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 87.7 dBA for the 445 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 35 minutes not sample.     e. An employee in the production area was exposed to continuous noise at 86.9% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 89.0 dBA for the 463 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 17 minutes not sample.     f. An employee in the production area was exposed to continuous noise at 80.9% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 88.5 dBA for the 458 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 22 minutes not sample.     g. An employee in the production area was exposed to continuous noise at 114.2% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 91.0 dBA for the 451 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 29 minutes not sample     h. An employee in the production area was exposed to continuous noise at 80.7% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 88.5 dBA for the 457 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 23 minutes not sample     i. An employee in the production area was exposed to continuous noise at 83.8% of the permissible daily dose level, when measured with a 80 dB threshold, or equivalent sound level of approximately 88.7 dBA for the 433 minute sampling time on November 5, 2015. Exposure calculation included a zero increment for the 47 minutes not sample     The employer had not implemented an adequate audiometric testing program.
