The attachments showed signs of deterioration, and thus the potential for operators, helpers and others in the immediate work area to be exposed to a struck-by hazard if the load were to fall existed:      Below the hook lifting attachments (shoes) fabricated at the facility were not load test, and identification markings such as, but not limited to, rated capacity, lifting device weight (etc.) were absent from the connectors. On or about 12/01/2015, employees in the process of forming a part were observed using attachments (shoes) to lift, position and move sheet/plate metal stock that weigh approximately 500 (+) lbs.  The operator, helper and others in the immediate work area were exposed to a potential struck-by and/or crushing hazard should the load be inadvertently dropped or fall.
