Myroslav Omelchenko dba Myroslav Omelchenko, 639 East 36th Street, Erie, PA 16504, on or about November 5, 2015 and times prior thereto: Employees preforming work tasks including, but not limited to, cutting wood, are provided with and utilizing eye protection. Employees have not been trained on the proper use, care, and maintenance of personal protective equipment. B.) Myroslav Omelchenko dba Myroslav Omelchenko, 639 East 36th Street, Erie, PA 16504, on or about November 5, 2015 and times prior thereto: Employees preforming work tasks including, but not limited to, cutting wood, are provided with, and utilizing, flexible extension cords and electrically power circular saws. In addition, employees are performing roofing activities including, but not limited to, laying down roofing paper while in proximity to the overhead electrical power line coming into the structure. Employees have not been trained on electrical safety work practices including, but not limited to, the use of GFCIs, recognizing hazards of damage to flexible cords, and recognizing hazards of overhead power lines. C.) Myroslav Omelchenko dba Myroslav Omelchenko, 639 East 36th Street, Erie, PA 16504, on or about November 5, 2015 and times prior thereto: Employees are provided with a Kidde K20M dry chemical fire extinguisher (S/N: VZ-65229). Employees have not been trained on the proper use of the dry chemical fire extinguishers.

The fire extinguishers last annual inspection was in January 2010. B.) Myroslav Omelchenko dba Myroslav Omelchenko, 639 East 36th Street, Erie, PA 16504, equipment trailer, on or about November 5, 2015 and times prior thereto: Employees are provided with a Kidde K20M dry chemical fire extinguisher (S/N: VZ-65229). Monthly inspections of the fire extinguisher were inadequate in that management failed to identify the expired annual inspection and the missing pin in the fire extinguisher.
