29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  IN PAINT MIXING, SPRAY BOOTH, AND SANDING/BUFFING AREA: On or about November 11, 2015, and at times prior thereto, employer requires employees to wear respiratory protection while painting trailers and trucks without a respiratory program to address key issues such as but no limited to: maintenance, fit testing, and medical evaluation for employees required to wear respiratory protection.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  WAREHOUSE WORK AREA: On or about November 5, 2016, and at times prior thereto, employees were exposed to a caught-in hazards while operating the Delta Unisaw to cut wood and plastic for trailer repair without adequate guarding over the exposed saw blade.

29 CFR 1910.215(a)(4):  On offhand grinding machines, work rests were not used to support the work.  Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch:  a) WORKSHOP: On or about November 5, 2015, and at times prior thereto, employees were exposed to a struck-by hazard from utilizing the Delta Shopmaster grinder with a work support 1/8 inch from the grinding wheel.  b) WORKSHOP: On or about November 5, 2015, and at times prior thereto, employees were exposed to a struck-by hazard from utilizing the Delta 10 inch bench grinder with a missing work support.
