29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) King Hall Galley Kitchen - Metal receptacle outlets used as pendant hook ups for the hot carts were not listed for use, 208 volt, 15 amp service, on or about 11/03/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.334(a)(2)(ii): An employee used a defective or damaged item prior to tests necessary to render the equipment safe had been made after repairs: a) King Hall Galley Kitchen - An employee used a repaired Carter-Hoffman hot cart, model number PHW1822 and received a shock because the hot cart was not tested for a fault upon completion of the repair work, on or about 10/27/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
