29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about November 4, 2015, on the northeast side of a commercial roof, a supervisor was working beneath employees installing metal roofing sheets on the roof was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.

29 CFR 1910.178(m)(3): A safe place to ride was not provided where riding of trucks was authorized:  a.  On or about November 4, 2015 on the north side of a commercial building, employees were riding on the forks of the forklift, exposing employees to a fall hazard.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a forklift:  a.  On or about November 4, 2015 on the north side of a commercial building, the employer did not ensure that an employee was certified to operate a 6600 lbs. forklift, exposing employees to struck-by, and crushing hazards.  In the alternative:  1926.602(d)  Powered industrial truck operator training.  Note:  The requirements applicable to construction work under this paragraph are identical to those set forth at sub-section 1910.178(l) of this chapter.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about November 4, 2015 at a commercial construction building, employees working on the northeast side of a 5/12 pitch roof was not protected from a 25 feet fall hazard by the use of a personal fall protection system.
