29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On or about November 20, 2015 in the production area, where employees were exposed to a fire hazard when operating the converting line without keeping all places of employment, passageways, storerooms, and service rooms clean.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     On or about September 25, 2015, in the production area, where employees are exposed to caught in, struck-by and electrical hazards when performing service and maintenance on machinery without the an established energy control program.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    On or about November 4, 2015, at the re-winder, where employees were exposed to a struck-by hazard when using compressed air to clean fiber paper dust from the machinery without the employer having reduced the air pressure used for cleaning to less than 30 psi.
