29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on equipment (such as compressors, mixers, screens, or pumps used for concrete and masonry construction activities) where the inadvertent operation of the equipment was present: (a) The employer did not ensure that employees were prevented from performing maintenance duties on a concrete pump when the equipment was running and inadvertent cycling of the pump could occur. On or about November 4, 2015, and employee suffered an amputation when cleaning the hopper of a Putzmeister TS 2050 concrete pump.
