29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Jobsite, Across from 1265 McEwen Road, Canonsburg, PA:  On or about November 5, 2015, two employees were exposed to fall hazards because the employer did not have a training program for the employees to recognize and protect themselves against fall hazards.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:    a) Jobsite, Across from 1265 McEwen Road, Canonsburg, PA:  On or about November 5, 2015 2 employees were at the peak of the roof unloading squares of shingles being placed on the roof by the supply company's truck, and were not protected from falling.  The fall hazard to the employees was approximately 30 feet to the ground.    b) Jobsite, Across from 1265 McEwen Road, Canonsburg, PA:  On or about November 5, 2015, one employee was installing shingles on a residential roof and was not protected from falling.  The fall hazard to the employee was approximately 24 feet to the ground.    B. Guyaux Jr. Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1029.501(b)(13), which was contained in OSHA inspection number 899733, citation number 1, item number 1 and was affirmed as a final order on May 7, 2013, with respect to a workplace located at 722 Crosswinds Drive, McMurray, PA.
