29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:     Employees required to dump various containers of water in the old Uniform Country space and the offices in the Old Sears space were exposed to unsanitary conditions due to the lack of housekeeping resulting in damp conditions and surfaces with the presence of mold growth.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      Employees were exposed to fall hazards while performing roofing activities on top of the mall because of the lack of fall protection and the employer did not have a training program for providing fall protection for the employees.
