29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s)    a.  On or about October 27, 2015, the AAA-080 Folder Gluer Machine was not isolated at its energy source when the operator was exposed to nip points at the following locations:            1.  In-feed chain of the gluer cylinder section, and the        2.  Center in-turning cylinder where there is only a non-interlocking plexiglass shield when reaching in to apply or remove tape to the cylinder which pulls the folders through the line.     b.  On or about January 21, 2016, employee(s) operating the Kemp 137 folder line were exposed to the hazard of having a hand/fingers drawn into the in-turning cylinders when reaching in to affix the stamp, straighten the stamp, or remove the stamp on the cylinder because the energy source was not deenergized.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    a.    On or about October 27, 2015, the operator of the AAA-080 Folder Gluer Machine was exposed to nip points when not applying a lock to the energy source disconnect when:        1.  Retrieving the glue top or otherwise having hands in the chain in-feed section where glue is applied.        2.  Reaching in to apply or remove tape to the cylinder which pulls the folder through the line at the center cylinder section of the machine.    b.    On or about January 21, 2016, employee(s) operating the Kemp 137 folder line did not apply a lock to the energy source when exposed to the hazard of having a hand/fingers drawn into the in-turning cylinders when reaching in to affix the stamp straighten the stamp, or remove the stamp on the cylinder.
