On or about November 05, 2015, at the above addressed jobsite, employees performing roofing operations approximately forty (40) feet above the ground were not protected from free falls, thereby exposing employees to fall hazards.

29 CFR 1926.502(e)(3): Connectors shall be drop forged, pressed or formed steel, or made of equivalent materials:  a. On or about November 05, 2015, at the above addressed jobsite, employees were performing roofing operations with fall restraint systems. The connectors for the restraint systems were not drop forged, pressed or formed steel, thereby exposing employees to the hazards associated with falls.
