29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: On or about November 5, 2015, at the above addressed job site, employees were exposed to the risk of being trapped in the event of a fire or evacuation from an exit door in the break room with panic hardware that had a locked dead bolt during the hours of operation.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface. On or about November 5, 2015, at the above addressed job site, employees were exposed to a shock and electrocution hazard while operating an energized 110 VAC light switch which had been lacking a cover.
