29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.  a) 2500 Knights Road, Creekside Apartments - On or about November 6, 2015, employees performing roofing work are using a pneumatic nail gun without eye protection.  b) 2500 Knights Road, Creekside Apartments - On or about November 6, 2015, employees performing housekeeping work beneath areas where debris is being dropped from elevated work areas without wearing head protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  2500 Knights Road, Creekside Apartments - On or about November 6, 2015, employees are using a aluminum extension ladder to access the roof and it does not extend at least three feet above the upper landing surface.
