29 CFR 1926.503(c)(3): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Job site, 8040 Peebles Road, Pittsburgh, PA: On or about November 5, 2015, during metal trim and repair activities for a new roof system, the employer did not ensure that fall protection systems/devices were in use where an employee was exposed to a fall hazard of at least approximately 13 feet in that the employee worked outside the flags and no other form of fall protection, such as a safety monitor, harness, guardrail or other appropriate fall protection, was used.
