29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:  a)B & S Welding Service:  On or about November 3rd, 2015 and prior to, employee(s) were exposed to explosion and struck-by hazards from compressed oxygen tanks, mixed welding gas and acetylene tanks that did not have valve protection cap on stored full tanks exposing employees to explosion and fire hazards while changing tanks out.   Compressed Gas Association Pamphlet G-1-1966, 4.2.8 states Valve protection for in the design, should be mechanically attached except when cylinders are in actual use.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:   a)  Miller welder #1:   On or about November 3rd, 2015 the employer exposed employee(s) to electrical shock, burns and electrocution hazards from unguarded or uncovered welding leads from the working and electrode leads connected to Miller  480 volt welder machine.   b)  Lincoln welder #1:   On or about November 3rd, 2015 the employer exposed employee(s) to electrical shock, burns and electrocution hazards from unguarded or uncovered welding leads from the working and electrode leads connected to Lincoln 480 volt welder machine.
