29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Workplace, Packaging Department, 105 Harbor Drive, Jersey City, NJ Three of the four employees engaged in cleaning operations on Bagger Machine #1 failed to apply their own individual locks. During such activities workers were exposed to hazardous residual energy and sudden restart of equipment without their knowledge, resulting in serious injury. Violation observed on or about 11-3-15
