29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 6 November 2015, and at times prior thereto, two employees were allowed to perform framing work on a commercial building without the use of hard hats, exposing both workers to the hazard of being struck by falling debris, materials, or tools.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 6 November 2015, and at times prior thereto, two employees were utilizing pneumatic nail guns to perform framing work on a commercial building without the use of eye protection, exposing both workers to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 6 November 2015, and at times prior thereto, two employees were allowed to perform framing work on a commercial building while standing on the floor's top plate at the edge of the building, exposing both workers to a fall hazard of approximately 32 feet or more to the ground below.
