29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer is failing to protect employees from struck by and caught between hazards associated with trenching and excavations. The most recent occurrence of this was found on the job site located 1 block north of the corner of 166th Ave and Ida Street in Omaha, NE where the employer has unprotected employees boring under a cross road between two trenches each of which is greater than five feet deep, as they are preparing to install water lines.
