29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. For the Save a Lot establishment at 1211 Homestead Road N, Lehigh Acres, FL 33936, as observed on or about 10/16/2015, employees were exposed to laceration, contusion, and amputation hazards as a result of operation of the following machinery: a) A meat cutting band saw was operated with its working blade extended 3.5 inches above its meat guide resulting in the potential for employees to contact the unguarded, unused blade section. b) An AMFG 532 meat grinder/mixer had a top access lid that was routinely opened with the machine still running. As a result, an employee's arm was caught between the paddles resulting in a contusion. Employees were also exposed to amputation hazards from a meat conveying auger that could be contacted after opening the top access lid.

29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) For the establishment at 1211 Homestead Road N, Lehigh Acres, FL 33936, as observed on or about 10/16/2015, employees operated an electric pallet jack and certification record of their forklift training was not available.
