29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about November 4, 2015, at 918 Duluth Highway, Lawrenceville, Georgia:  Two employees performing welding work were exposed to fall hazards of approximately 20-feet and greater when they used a Genie S-40 and a JLG 460 SJ boom lift without tying off to the cage of their boom lifts.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 4, 2015, at 918 Duluth Highway, Lawrenceville, Georgia:  An employee performing welding work was exposed to a fall hazard of approximately 20-feet and greater when he used an extension ladder that was not extended three feet above the landing.
