29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   A) Job site, at/near 1709 Termon Avenue, Pittsburgh, PA: On or about November 7, 2015, during roofing activities, the employer did not ensure that fall protection was used where employees were exposed to falls of up to approximately 30 feet while working on a ladder jack scaffold.
