29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6:  417 S. 7th St., Seward, NE 68434 (Shipping, North Wall): The employer failed to ensure that the storage of two propane cylinders was adequate.  The cylinders were located by an employee pathway and the cylinders were not was secured while they were in storage.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):                                                           417 S. 7th St., Seward, NE 68434 (Site-wide):  There was no energy control program provided by the employer that met the requirements of 1910.147, including but not limited to;     a)     Authorized employees servicing equipment,  b)     Non-authorized employees whose work operations may be in a area where energy control procedures are being utilized and;  c)     Outside contractors engaged in activities covered by this standard.    Abatement certification and abatement documentation is required for this violation. The abatement certification sheet is enclosed with the citations.

Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
