29 CFR 1910.132(f)(1)(iv): Employee(s) required to use PPE by this section were not trained to know the limitations of PPE: a) On or about November 4, 2015 and at times prior, an employee fell approximately 25 feet from a telecommunication tower. The employee was wearing a personal fall arrest system that was hooked on a cross-brace of the tower. The employee was not properly instructed to the limitations of the PPE in which the cross-brace was used as an anchorage connector that was not capable of withstanding a 5,000-pound load as required in the ANSI Z359.1-2007 (Safety Requirements for Personal Fall Arrest Systems, Subsystems and Components) Section 3.2.5.1.

29 CFR 1910.268(g)(1): The employer did not ensure that safety belts and straps were provided and used when work was performed at positions more than 4 feet above the ground, on poles, and on towers: a) On or about November 4, 2015 and at times prior, an employee fell approximately 25 feet from a telecommunication tower. The employee's positioning device system was hooked on the cross-brace of the tower that was in the process of being removed. The employee fell as soon as the bolt was removed from the cross-brace. The supervisor was working with the employee on the tower and did not ensure that the positioning device system was used correctly.
