On 10/22/2015, employees from Reliable Fab & Manufacturing Inc. were working at 2235 Highway K, OFallon MO.  An employee was installing light fixtures on the front of a commercial building while in a JLG Aerial Lift, Model 400S / Serial #0300132676.  He was approximately sixteen feet above the ground and not wearing a body belt with lanyard attached to the boom or basket while working from the aerial lift.
