29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. On 10/19/2015, employees from Alliance Contracting were constructing a second story addition on a residential building located at 6937 Brassel Drive in O'Fallon MO. Employees were passing lumber through a wall opening of a second story addition, which was approximately twelve feet above the ground. The employees were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.

On 10/19/2015, employees from Alliance Contracting were constructing a second story addition on a residential building located at 6937 Brassel Drive in OFallon MO. An employee was carrying supplies up the ladder, which could have caused him to loose balance and fall.
