29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. (a) Located on the new build of the vessel "Active": On or about and at times prior to October 27, 2015 the inspection revealed that employees were potentially exposed to serious injury from falls to a lower level of about 7 feet due to open hatches that were not covered or guarded.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: (a) On or about and at times prior to October 27, 2015 the inspection revealed that the employer had not conducted industrial hygiene sampling to determine employee exposure levels to chemicals including styrene when repairing fiberglass sections on the new build of the vessel "Active." Located at 5555 N Channel Ave in Portland, OR.
