29 CFR 1910.26(c)(2)(iv) as required by 1960.8(b): Ladder(s) were not maintained in a good usable condition at all times:  In the hanger, an unmarked, three-step, special purpose stepstool ladder was not in usable condition in that the platform was not maintained with a nonslip coating and two of the ladder feet were not made of or covered with slip-resistant material.  Employees using the ladder during routine maintenance of planes were exposed to slip and fall hazards.

29 CFR 1910.151(c) as required by 1960.8(b): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  In the hanger, employees were required to mix, pour, and use chemicals including, but not limited to T-5 NF, Toilet Deodorant/Detergent during aircraft maintenance and repair operations, and the employer failed to ensure that suitable facilities for drenching or flushing the eyes were available in the immediate vicinity of the hazard.  The provided eye wash was not suitable in that it had not been inspected and operated on a weekly basis.

29 CFR 1910.1200(f)(6)(ii) as required by 1960.8(b): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  In the hanger, the employer failed to ensure that a plastic container of T-5 NF, Toilet Deodorant/Detergent was labeled with the product identifier and hazard warnings which exposed employees to irritation of the skin and eye(s).  The Federal Aviation Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(f)(5), which was contained in OSHA inspection number 944686, citation number 1, item number 6a and was affirmed as a final order on November 26, 2013, with respect to a workplace located in Anchorage, Alaska.
