29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about November 10, 2015, at the above addressed job site, an employee was exposed to a fall hazard of approximately 11 feet while sitting on top of a masonry wall while installing electrical conduit without a means of conventional fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 10, 2015, at the above addressed job site, an employee who had not been provided training to recognize fall hazards was exposed to a fall hazard of about 11 feet while sitting on top of a masonry wall while installing electrical conduit without a means of conventional fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the landing surface.  On or about November 10, 2015, at the above addressed job site, an employee was exposed to a fall hazard of approximately 11 feet, while using an extension ladder that did not extend 3 feet above the upper landing to access the top of a concrete wall while installing electrical conduit.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards.  On or about November 10, 2015, at the above addressed job site, an employee who had used an extension ladder that did not extend above the working surface, and was exposed to a fall of approximately 11 feet, had not been trained by a competent person in the proper, use, and placement of ladders.
