29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a) Production Work Area: On or about November 30, 2015, an employee was trained to use the company's machine maintenance-lockout / tagout procedures, which included a step that required the employee to conduct voltage meter testing of potentially energized circuits, but protective clothing, shields and barriers was not utilized wherever it was necessary, exposing employees to potential injury from electric shock or arc flash.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control.  a) Production Work Area: On or about November 30, 2015, an employee was trained to use the company's machine maintenance lockout / tagout procedures on equipment with multiple energy sources, but the listed procedures did not clearly and specifically outline techniques to be utilized to prevent injury from the unexpected release of stored energies.
