29 CFR 1928.57(a)(6)(iii): The employer did not instruct every employee to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: (a) West end of center bay in Production Building - on or around November 5, 2015 an employee working at the Moss Lint Cleaning Machine failed to disconnect the power source and wait for all moving parts to stop prior to unclogging the machine resulting in the amputation of the tip of middle finger on the right hand.

29 CFR 1928.57(a)(6)(v): At the time of initial assignment and annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with whick he is or will be involved, including locking out electrical power before performing maintenance on farmstead equipment: (a) West end of center bay - On or about November 4, 2015 an employee was clearing the clogged Moss Lint Cleaner machine by asking the assistant manager to shut off the Moss Lint Cleaner machine. The assistant manager shut off the machine at the control panel without performing proper Lockout /Tagout procedures to control the stored Kinetic energy in the cutting blade rollers resulting in employee having the tip of his right middle finger amputated.

29 CFR 1928.57(d)(1)(iv): Pulleys of V-belt drives shall be completely enclosed or guarded by location whether or not railings are present. The open end of the pulley guard shall be not less than 4 inches from the periphery of the pulleys. (a) West wall in center bay - on or about November 5, 2015 employee's were operating the Moss Lint Cleaner machine that had no guards around the V-belt drives, and pulleys to protect employees from pinch points that could result in amputations.

29 CFR 1928.57(c)(5)(ii)(B): No electrical disconnect switch was not provided to the employee(s) within 15 feet of the equipment upon which maintenance or service was being performed: (a) West wall in center bay in Production Area - On or about November 5, 2015 employees were operating and cleaning the Moss Lint Cleaner Machine that had no electrical disconnect switch within 15 feet of the machine. Moss Lint Cleaner machine shut off switch is located around the corner of other machines 50 feet away.
