29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      November 12, 2015 employees were not protected from a fall while walking/working on the roof of a commercial building, exposing employees to a fall of over 25 feet.      Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     On or about November 12, 2015 employees working at heights were not trained to recognize or minimize the hazards associated with a fall, exposing employees to a fall of over 25 feet.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:     On November 12, 2015 an employee was using an "A" frame ladder in the closed position, exposing the employee to the hazards associated with improper ladder use.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
