The out-feed section just past the chainsaw had a nip point at the end of the conveyor.    b.  On or about February 8, 2016 at the facility located N5531 St. Highway 76 in Shiocton, WI the employer did not have adequate guarding of the chain conveyor in the Scrag Room.  The in-feed section just before the chainsaw did not have an adequate guard over the chain and sprocket driving the chain.    c.  On or about February 8, 2016 at the facility located N5531 St. Highway 76 in Shiocton, WI the employer did not have adequate guarding of the chains and sprockets of the Three Strand Cant Conveyor located in the Resaw room.    d.  On or about February 8, 2016 at the facility located at N5531 State Highway 76 in Shiocton, WI the employer did not have adequate guarding of the  Melot chain conveyor in the Resaw room.  The Melot Chain Conveyor had unguarded sprockets on the underside of the table.    e.  On or about February 8, 2016 at the facility located at N5531 State Highway 76 in Shiocton, WI the employer did not have adequate guarding of drive chains on the the 6 Head Grading Table conveyor in the Resaw Room.  The conveyor had multiple unguarded drive sprockets.    f.  On or about February 8, 2016 at the facility located at N5531 State Highway 76 in Shiocton, WI the employer did not have adequate guarding of the chain conveyors in the Resaw Room by the End Trim Operators workstation.  The conveyor had unguarded drive sprockets.    g.  On or about February 8, 2016 at the facility located at N5531 State Highway 76 in Shiocton, WI the employer did not have adequate guarding of the chain conveyor where the employee stacked product by beginning of the Line 2 Resaw Line.  The employee was exposed to a nip point at the end of the chain conveyor.    h. On or about February 8, 2016 at the facility located at N5531 State Highway 76 in Shiocton, WI the employer did not have adequate guarding for the belt and pulley for the In-feed Drive conveyor of the Slab Edger that was located approximately 79 inches above the floor.    i.  On or about February 8, 2016 at the facility located N5531 St. Highway 76 in Shiocton, WI the employer did not have adequate guarding of the vertical arm conveyor in the Resaw Room.  The conveyor that stacked lumber at the end of the grading table did not have guards covering the drive chains section on the South side of the machine.
