29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes: (a) On or about February 2, 2017, pulleys powering the shaker conveyor located in the scrag building were not guarded to prevent employees from accessing the rotating parts and ingoing nip points. (b) On or about February 16, 2017, pulleys powering the riser pit barn cleaner in the scrag building were not guarded to prevent employees from accessing the rotating parts and ingoing nip points.

(a) On or about February 2, 2017, an inclined belt powering the shaker conveyor located in the scrag building was not guarded to prevent employees from accessing the rotating parts and ingoing nip points. (b) On or about February 16, 2017, an inclined belt powering the riser pit barn cleaner in the scrag building was not guarded to prevent employees from accessing the rotating parts and ingoing nip points.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the considerations listed in 1910.303(b)(1)(i)-(viii). (a) On or about February 16, 2017, a main junction box located near the ceiling in the north east corner of the scrag room did not have a cover, exposing employees to potential fire hazards from the buildup of sawdust in the box and on the electrical connections. (b) On or about February 16, 2017, an outlet powering the lift table for the re-saw feeder was damaged resulting in the outlet cover not effectively closing the receptacle.

Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable: On or about February 16, 2017, employees were exposed to fire hazards created by the excessive buildup of sawdust and wood debris in the Scrag saw building around the shaker conveyor and edger.
