29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. In the machine shop, employees were exposed to amputation hazards, in that, wile conducting maintenance on the bridge port milling machines one through four, precision CNC machines one through three and, the vertical band saw there was no program to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative, on or about 11/12/2015.

In the machine shop, employees were exposed to struck by and amputation hazards, in that, the three Bridge Port Milling machines model #251, Arcadia Mill milling machine  SN #1025, and the Rockwell Delta drill press were not guarded while drilling holes in aluminum, on or about 11/12/2015.
