Location - 103 Elm Street, Pepperell, MA On or About 11/12/2015, a competent person did not perform frequent and regular inspections of the job site to identify safety and health hazards. The fall hazards should have been corrected during inspections.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. Location - 103 Elm Street, Pepperell, MA. On or about 11/12/2015, an employee fell greater than 21 feet 5 inches in height to lower level from a residential roof while walking/working were protection was not used. MMP Construction, LLC was previously cited for a violation of this 29 CFR 1926.501(b)(13) occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number #315147298 citation #01 item/group#001 and was affirmed as a final order on 4/25/2012, with respect to a workplace located at 81 Coolidge Avenue, Needham, MA.
