29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section: At the jobsite located at 15505 Larsen Street in Overland Park, Kansas: Employees engaged in residential roofing activities were exposed to head injury from falling objects. Broken parts of the terra cotta roofing tiles were being thrown from the roof while employees on the ground, such as the all terrain fork truck operator, were not wearing head protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsite located at 15505 Larsen Street in Overland Park, Kansas: Employees engaged in residential roofing activities were exposed to fall hazards of approximately 14 feet, 6 inches from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.
