29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:     a) Extrusion Department, Extrusion Line Augers.  On or about November 12, 2015 and thereafter, employees performed servicing and maintenance tasks on extrusion lines when performing product changeovers.  Employees who cleared resin from the magnet areas by hand were exposed to the moving auger blades beneath a 3" feed throat opening through which the resin flowed.  Employees were not required to control hazardous energy sources in the following sequence:       1) turn off and shut down the machines,     2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),   3) affix lockout or tagout devices to each energy isolating device,     4) verify that isolation and deenergization of the machines or equipment have been accomplished.       Employees were exposed to caught-in hazards likely to result in finger laceration and amputation hazards from unexpected start up and/or energy release of the extrusion augers.   Alternative protection, such as machine guarding, was not adequate.     There is no abatement certification or documentation required for this item.
