OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from a rollover or from the operator falling out due to the lack of an operator restraint system (seat belt) provided on a powered industrial truck (forklift). a) Worksite, P.O. Box 158 Industrial Park Road, St. Clair, PA - Employees operated the Halla Lift Truck Model LF-30 (Serial No. 1041K) with roll over protection (ROPS) that lacked an operator restraint system (seat belt) which exposed employees to crushed-by hazards in the event of an accident, on or about November 12, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Box 158, Industrial Park Road, Saint Clair, PA - The exit lighting for every exit sign was not operating due to deficiencies such as but not limited to, missing light bulbs, broken light bulbs, and no electrical power exposing employees to hazards associated with emergency evacuations, as observed on or about November 12, 2015. b) Northeast Wall, Red Double Door Emergency Exit, Main Bay, P.O. Box 158, Industrial Park Road, Saint Clair, PA - The panic exit device cross bar on the right hand door was broken exposing employees to hazards associated with emergency evacuations, as observed on or about November 12, 2015. c) Warehouse, P.O. Box 158, Industrial Park Road, Saint Clair, PA - The exit lighting for every exit sign was not operating due to deficiencies such as but not limited to, missing light bulbs, broken light bulbs, and no electrical power exposing employees to hazards associated with emergency evacuations, as observed on or about November 12, 2015.
