29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about October 28, 2015, at the job site employees  were installing roofing materials on a roof more than 6 feet above a lower level.  The employees were not  wearing personal fall arrest and had no other type of fall protection system provided.  Employees were exposed to fall hazards.    Paco Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 281796, citation number 1, item number 1, and was affirmed as a final order on June 29, 2012 with a final abatement date of  September 25, 2012, with respect to a workplace hazard at 1900 Pinehurst Trail Drive, Oklahoma City, OK 73107.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about October 28, 2015, at the job site employees were observed not wearing safety glasses while using hammers and pneumatic nail guns to install roofing materials.  Employees were exposed to eye injury hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about October 28 2015, at the job site employees were observed using portable ladders to access the roof that the side rails did not extend at least 3 feet (.9 m) above the upper  landing surface and were not tied off at the top.  Employees were exposed to fall hazards.
