29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On or about November 12, 2015, at the job site employees were allowed to work from a personnel lifting platform mounted to a forklift truck without written approval. The platform was not secured to the carriage or forks and the width extended more than 10 inches beyond the tires on each side. Employees were exposed to fall and struck by hazards.
