29 CFR 1926.453(b)(2)(v): A body harness with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift:      Apartment building under construction; East Side, Genie Aerial Lift (Model: S80, Equipment Rental Number: 148110), On or about November 12, 2015, an employee was exposed to fall hazards of approximately 30 feet in height to the unpaved ground below, when installing sheet rock from an aerial lift without the use of a lanyard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    Apartment building under construction, East Side, Genie Aerial Lift (Model: S80, Equipment Rental Number: 148110), On or about November 12, 2015, an employee was exposed to fall hazards greater than 6 feet in height to the unpaved ground below, while climbing out of an aerial lift and into a window opening without fall protection.
