29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) projected more than one half the diameter of the shaft:    a) Facility, Conveyor to grinder: A rotating shaft approximately 1 inch in diameter projected out approximately 1 inch, on or about 9/25/15.    b) Facility, MPO, Oven Conveyor to CPLO#2: A rotating shaft approximately 1 1/4 inches in diameter projected out approximately 1 inch, on or about 9/25/15.    c) Facility, CPLO#2: A rotating shaft approximately 1 1/4 inches in diameter projected out approximately 2 1/4 inches, on or about 9/25/15.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
